Randomised trial of the clinical and cost effectiveness of small bore, Seldinger, versus large bore, surgical, chest drains for the treatment of traumatic haemo/pneumothoraces.
CoMITED-2: Is Bigger Always Better?
Chest injuries can cause air or blood to collect around the lung, often requiring a chest drain to help the lung recover. For many years, large surgical chest drains have been the standard treatment—but could a smaller, less invasive drain offer better outcomes?
CoMITED-2 is a UK-wide clinical trial comparing small-bore (≤14Fr) Seldinger drains (inserted using a needle and wire) with large-bore (≥24Fr) surgical chest drains in adults with traumatic pneumothorax, haemothorax and haemopneumothorax.
The study will investigate whether smaller drains are safer, more effective, improve patient recovery, and provide better value for the NHS.
With around 1,600 patients across 50 NHS hospitals, CoMITED-2 will provide important evidence to guide future trauma care.

The Edinburgh team is led by Dr Lyle Moncur, Principal Investigator, with Val McDowell, Trauma Advanced Nurse Practitioner, as Associate Principal Investigator, and Alison Grant as Study Coordinator.
